Effectively using a Care Notebook facilitates the coordination of care between parents/caregivers and their child’s many different providers and services. A Care Notebook empowers families and opens lines of communication between everyone involved in the care of your child.
Be sure to bring your child’s Care Notebook with you to all your child’s appointments, including specialists, diagnostic tests, physical and occupational therapies, educational IEPs and any other meeting relative to your child’s special needs.
